We have an open list and welcome requests for registration from patients living in or moving to the practice area. A handy map to check your location and a list of the villages included in our practice area can be found here
If you live within our Practice area you are welcome to register with us and our reception staff will be happy to guide you through this procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of Photographic identification.
How you can register
You can register by completing the forms available from reception or as downloads.
Please download and complete (by hand) a GMS1 Registration Form for each patient.
You will also need to complete a Registration Form and drop off a list of medication you are currently taking. Adult Registration Pack 2023

Please complete the above forms and bring to reception with proof of address and a form of photographic identification.

You will have a named, accountable doctor who is responsible for coordinating your care. You may see any of the doctors and can express a preference for which doctor you want to see, subject to availability.
You will be invited to make a New Patient Appointment which we ask you to bring a list of medication you are currently taking.
