Repeat prescriptions can be ordered by completing the right-hand section of your previous prescription form and either leaving it at the surgery or sending it to us by post. You can also order repeat prescriptions online using this form below:

Your prescription will be ready for collection 5 working days later. If you prefer, we can post it back to you (you will need to provide a stamped, addressed envelope).
We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed you will be asked to make an appointment.
Prescription Exemption
If you have a long-term condition, you may be eligible for free NHS prescriptions.
You can check if you are eligible here.
If you are entitled to free prescriptions, please collect an FP92A (Prescription Exemption Application Form) from Dispensary, fill it in and then return it to reception. This form will then be completed by a GP or medical representative who has access to your medical records and they will send the form off on your behalf.
You will then receive your exemption certificate in the post – so please make sure that your contact information is correct and up to date.
If you are pregnant you may also be eligible for free NHS prescriptions. You can collect a FW9 (Maternity Prescription Exemption Application Form) from the surgery and follow the same steps as above.
Medication Review Request
We review any regular medication on a repeat prescription annually and wherever possible the doctor will do this without you having to attend the surgery.
If you have been advised by the surgery that your medication review is due please use this form. This process can take up to 10 days. To request a medication review please use this form
